1988 GMC C1500 Dies when not on throttle

I was driving when all the sudden the engine wouldnt accelerate and finally died. In order to restart the engine I have to have the pedal to full throttle or it just keeps cranking. If I dont have some throttle it will died, if its in neutral it dies and it accelerates up to 40 mph very rough. Yet over 40 mph it runs great. Also, there is white/grey smoke from the tailpipe. Please help
