Hello,
It sounds like the issue could be the fuel level sending unit. The sending unit consists of a float and a resistor panel. Many times the resistor goes bad and that can lead to an incorrect fuel gauge reading. The level sensor is mounted on the fuel pump assembly and yes you will have to remove the fuel tank to access it.
I have attached the procedure below for your reference.
Another possibility would be faulty gauge or wiring issue.
