2002 Ford Windstar oil in coolant overflow

2002 Ford Windstar 6 cyl Front Wheel Drive Automatic 78k miles

Is a failed head gasket the only reason for this to happen? I also have P0171 and p0174 codes. (lean on bank 1 and bank 2. Could this have caused the problem?

That is just about the only way for it to get in there, unless oil was dumped in there.

the o2 sensor codes could be caused due to the headgasket.
