2001 Ford Windstar twice I get trouble code PO 136

I got trouble code po136 downstream heated O2 sensor fault(Bank 1) hand it replaced and one month latter my check engin light is back on and I checked andit reads PO136. What would be the problem?

P0136 deals with the circuit, not necessarily the sensor itself.
