1999 Ford Windstar Engine oil in radiator

I recently replaced head gasket on my van because it had engine oil in radiator, after I was done I check again the radiator and there was oil in it again, what could be the problem?

have the cooling system flushed and refilled .. if the oil returns .. suspect headgasket still at fault
