1992 Ford Truck Stumbles under load

If I accelerate very slowly the truck goes smoothly but when climbing a grade or speeding up more quickly it stumbles alot it will down shift if I accelerate harder but still stumbles alot I just replaced the fuel filter but havent tried tuning it up yet.
