Belts don't fall off on their own. It's nearly always due to some type of alignment issue that is usually triggered by a failing pulley or component that starts to lean under pressure and the belt just migrates off the edge.

How often does it happen? Can you standby and watch it happen? Are there any odd noises you can hear when this happens or before? Have you looked/tested for any loose components that carry the belt? The usual suspect would be the idler pulley assembly which applies tension to the belt. Also the condition of the belt itself must be taken into consideration.
