1992 Ford Tempo replacing a pulley

a small smooth pulley fell off my car, it has four bolt holes to attach it to where it fell off, it is located just underneath the alternator pulley.
The serpentine belt did not break, but all the bolts are missing to the pulley, what may have caused this ?

the power steering pump should be under the alternator and it has a double pulley. 1 for serpentine belt and the other for the alternator. sounds like the alternator belt pulley came off. that does seem strange! you could put new bolts with lock washers to make sure it doesn't happen again?
