1997 Ford Taurus water pump

my serpentine belt schredded the other day, and the pulley on the water pump unscrewed. I tightened the pulley, and replaced the belt, but when I started the car again, the new belt schredded, and the pulley unscrewed again. What could cause this?

faulty pulley will cause the belt to twist and get skewed / shredded. most likely the water pump pulley since you say it was loose. it will need replaced.
