1997 Ford Taurus rough shift to 2 gear won't shift into hig

had the transmission (AX4N) rebuilt about 3 years ago, now it has started shifting rough into 2nd gear and will not shift into high gear. Sometimes when stopped but still in drive feels like it wants to lunge then RMP jump un and down then it dies. Also, does this when slowing down to make a turn. A friend suggested that it could possible be a loose vacuum hose, but not sure.

It sounds like a couple problems. First, is the trans slipping or just shifting hard? Is it shifting at the right times?

Next, does the engine surge at an idle or anytime?
