The ability to reverse my vehicle went about 6 months after I bought it. To park it I have to put down the emergency break. To start it I have to put the vehicle in Neutral. I took it into a mechanic who tightened a screw in the gear shifting compartment. The screw comes loose within the same week but I'm still able to reverse, park and drive the vehicle until it just gets too loose where I can't reverse it and I have to take it in every 2 months to get it tightened again.

What is the fix for this problem and how much would it cost.
