it sounds like you may have a transfrecase problem. is this an electric shift transfercase.when you push in on the clucth all rotation in the transmission stops that is why the noise stops. have you had someone check all the fluids in the transmission and transfercase?(NOS) 8)

Suggested Repair

Have the transmission and transfer case fluid levels and condition checked, and inspect the transfer case. The noise stopping with the clutch depressed points to a rotating drivetrain component, and the expert suspects a transfer case problem.

Conversation Summary

The owner of a 1985 Ford Ranger XLT 4x4 reports a loud noise like something is loose in neutral and in 1st, 2nd, 3rd, and 5th gears, but the truck is quiet in 4th gear and with the clutch depressed. The expert suspects a transfer case problem, notes that pressing the clutch stops transmission rotation and therefore the noise, and recommends checking the transmission and transfer case fluids.
