1999 Ford Ranger Rough running when cold

Before I buy this well cared-for truck, I'd like your thoughts on the trucks' rough running, seemingly on 5 cylinders until it warms up, then all is well. When cold it also has poor acceleration. Once its all warmed up, its great!

No smoke, no leaks, no warning lights.

Car Fax shows Ford dealer only service for its entire life and no crashes.

At this mileage is the end near for this engine?

How much? Missing when cold does not sound promising, but advice on buing new cars is not easily given

Although it doesn't have a check engine light, I'd check for pending codes.
There are a few tests you can perform before buying the car (engine vacuum, fuel pressure, compression, etc). All these tests can tell you a lot about the shape of the engine.
Best case scenario, the problem comes from a bad sensor (IAT, ECT?), You replace it, problem solved.

I'd do a little bit of research online to find a guideline on buying a used car.

By the way, if the truck has been maintained well, 166000 miles is not too bad. You should be able to get a little more out of it. (I drive a 1990 Ranger with 250000 maybe 350000 miles on it. Still runs strong!).
