1998 Ford Ranger idle and acceleration

When I start the car from cold initially, it starts , but it idles very erradic, and allmost stalls. this continues for about minute, the smoothes out and idles normally, but will rev up above idle. If I press and hold the accelerator down the engine begins to rev, but cuts out like it is stalling. It then revs up for a second, but begins to stall again, then contiues to do this over and over for as long as the throttle is held down. When I let of the gas the engine stalls completely. I have retrieved 3 codes, and replaced three parts, none have had any affect. the TPS, IACS, and PCV. I have disconnected the exhaust at the maniflold to see if the exhaust is plugged, I have checked that the fuel pump is operating, it is. What is causing this, can you enlighten me???

Check the actual fuel pressure using a fuel pressure tester designed for your fuel system, also test the coolant temperature/MAF sensor, PCV and EGR valves-
