2001 Other Ford Models Car gets hot and the tail pipe has wh

I would like to know what the mechanical problem could be. The car over heats. It was driven for a while like that with water being added to the radiator every few days. We have changed a broken belt and the thermostat. It was better for a while. Now the car is over heating and it has white smoke coming from the muffler. What is your opinion?
