the power steering belt keeps slipping off is it time to replace belt or could it be other problems the belt looks fine

Belt is probably stretched due to wear/tear overtime. Do any pulleys seem to wobble any?

Not sure of your exact area's pricing but the average in U.S. For replacing your vehicle's PS drive belt would be approx. $50 labor plus the cost of new belt.
