My 91 explorer wont shift out of (what seems like) 1st gear until I reach about 4000 rpms. It has plenty of power during this time and the vehicle is moving just fine, but it wont shift. It seems to happen more when it is cold. What could this be?

This could be bad oil. Or you gears are warn, your timing is off. My bronco used to do the same thing but won't shift unless you get off the gas, this happened because my oil was burnt.

Is the fluid low? Check it according to dipstick specs.
