1996 Ford Explorer sensor

Would anybody have any type of advice in changing a sensor fault. The sensor is

po136 sensor fault (bank 1 sensor 2)

po141 02 sensor heater fault (bank 1 sensor 2)

The sensor in question is the passenger side, rear sensor (after cat converter). But with both of those codes present, I would also inspect the wiring as well as the connector.
