1993 Ford Explorer motor running hot

Boiling over to raditor, and running hot.I have changed the T-stat. Need help!

Check radiator temp top to bottom if bottom cool middle warm top hot then rad stopped up. Check fan clutch should be hard to turn engine hot. Any coolant loss? Oil look normal?
