When engine is cold it surges (not like a miss) for about 2 miles. Then it immediately stops surging and runs very smoothly from then on. No engine light on. Does not feel at all like a defective plug coil as I have experienced that type of miss on several occasions when different coils have been defective over the life of the car. What causes the surges?

If it only happen when the engine is cold, it's probably the ECT(engine coolant temp sensor).
Can also be IAC, MAF Or TPS, try unplugging the MAF befor you start the car, if it smoothes out, replace the MAF.
