2000 Ford E-Series Van Overheating Problem

I was given a 2000 Ford Econoline Van with a V8. It has 400,000 K and runs really good. The only thing wrong with it is that it overheats in a short period of time (10 Minutes driving time) It will seem fine and suddenly Red Lines on the Temperature Guage. I will smell coolant and steam comes into the cabin and out under the hood.
It will start to lose power and sputter. Shut it down, let it cool for a few hours and then start again... Same Scenario. I suspect the fan is not engaging because I don't hear it. Could it be that or a thermostat problem?
