2004 Ford Crown Victoria i wonder whats the problem

when i put my 2004 crown victoria lx in drive it sometimes jerks pretty hard and when i increase speed sometimes hesitates to changed into he next gear

Any transmission problem like this should be referred to a trans specialist for a test and report, they will have all the specialist testing equipment to check for any fault.
