What is the problem when water leaks from the exhaust pipe and the car runs hot?

If it is antifreeze running out of the pipes, the head gaskets are shot. If it is clear or black water, that is just condensation. If it is the latter, copuld be a plugged radiator, bad thermostat, bad water pump, bad cooling fan.
