Fan belt auto adjustment unit rattle

We have fitted three new fan belt automatic tensioner units over the past six months and it is OK for a couple of days, it then starts to rattle, and you can see the tensioner oscillating on tick-over, it becomes very noisy, but can't be heard when driving, Any idea's,

if the belt was changed it should have been something may be out of line as well. have a pro check alignment of pulleys
