That is a good question. By chance, do you know if the flange in question is on the manifold or the Y-pipe?
If you look at the pics below, I highlighted the flanges on each component. There are studs that thread into the exhaust manifold flange. On the opposite end, the Y pipe, there are flanges that will fit over the studs on the manifold and then a nut is used to attach them.
If the manifold threads are bad and the stud won't stay in, if there is room, you can simply get a longer bolt and nut to reach through the manifold and y-pipe. If it is the flange on the Y pipe, that is installed by the manufacturer before the converters are welded on, so that is more of an issue.
