First thign I would do is pull the IAC module, clean it up good and reattach, clean throttle body/plate out and then let it run. Any better now? You can try seafoam in a vaccum lin also that cleans her up good.

If that is not the solution then you should check for vaccum leaks, faulty pcv valve, sticking egr valve, or a bad IAC module.
