2002 Dodge Ram windows icing up in winter while driving

Heater problem
2002 Dodge Ram 71000 miles

we had our heater core replaced last year because of our windows icing up in winter. We are having the same problem when the temp is really cold. We have to scrape the windows to see out.

When running the defroster, do you have the fresh air selected or recirculate?

It is set on defrost; I don't have a fresh or recirc option. Also, my windows are clear when I enter the van, but fog then ice-up once the van is on and the defrost fan is on. All the windows in the van frost to the point of needing to scrape to see out while driving.

The air is taken into the vehicle from outside (cowl). By any chance is it plugged?
