2002 Dodge Ram Oxygen Sensor troubles..

Ok.. So I have a 2002 dodge ram 1500 sport.. And I have been having troubles wih the oxygen sensor.. I went to autozone to do the free code reader and the code read one of the oxygen sensors wasn't getting enough oxygen.. I took it to midas and paid the $80 to get it fixed and they said it was no biggy just a leak in a vacume hose.. They fixed it they said. 3 Days later the check engine/emission light is on again.. which is exactly the same thing that happens when I disconect the battery.. Light goes off.. Resets code.. 3 days later roughly it is back on..

last time I had my oil changed.. Last week.. The guy told me the factory (chrysler/dodge) has an 8 year warranty on the oxygen sensor.. So I guess this is a 2 part question.. Anyone heard of the 8 year factory warranty on an oxygen sensor, how would I find that out? And the 2nd part.. Does this sound like I need a new oxygen sensor?? Let me know what you think

Have to have the codes
I am sorry but I have no idea and never heard of the 8 years warranty on the O2 sensors
