1997 Dodge Neon gear shifter issues

My car keeps jumping out of gear mainly 3rd and 5th gear. Can you tell me the most logical reason for this and how to repair the problem?
ok i tried what you suggested and adjusted the play in it but it is still jumping out of gear and the fluid is ok. Do you have any other suggestions for me?

Check the fluid to make sure it is clean and full. Also, check the shifter linkage to make sure there isn't excessive play in it.
