when I am driving for more than a couple of miles at a time, my car starts to jerk really bad, then it will lose all acceleration even if my foot is still on the gas, sometimes it will jerk again and my acceleration will come back other times it won't, also after it starts jerking if I apply my breaks my car stalls, it usually starts right back up, but sometimes it takes awhile, also if I have not drove my car in like a day, its sometimes hard to start my car, it turns over, but sometimes doesn't start for a good 15 min, any advice on what I should check would be helpful

I would first check two things. The fuel pressure needs to be checked when the problem is occurring, and make sure there is no damage to the exhaust system that could cause blockage or a plugged catylatic converter.
