Serpentine Belt

1997 Dodge Grande Caravan SE w/ 145,000 miles
The belt keeps falling off - primarily when it is wet out. Within the past 2 months the belt has been replaced 3 times and I have had to put it back on at least 10 times. Also, have replaced the tensioner and water pump and inspected the motor mounts. At times I get a high pitched noise that appears to be coming from the tensioner but it is infrequent and seems to go away after the car gets warm. Even my mechanic is scratching his head over this one. My brother in law says these vans have this problem when it's wet so I have gone as far as repairing the split spray shied to try to keep everything as dry as possible. One last thing - the problem started after hitting a rather large pot hole that was full of water with both front wheeels at about 10/15 mph. Any help will be greatly appreciated!

There is really only one logical solution. There is some one of the components driven by the belt that is not perfectly lined up, it is close. Find it and your problem will go away.
