2000 Dodge Caravan jumping when accelerating and cutting of

i have a dodge carvan and when you accelerate it tends to do some jumping and acting like it wants to cut off when your driving, however when you stop at stoplights or even if i m backing up and go to switch gears it shuts off, but you can put it in neutral and recrank it with no problem. i have checked the machine and it first said map sensor high voltage and the next time it read 1st gear ratio incorrect, everytime it pops a different code. i have replaced the map sensor, a new fuel pump, fuel filter, (about 1 year ago). i dunno whats wrong. any ideads?
