1998 Dodge Caravan V6 3.0 - Shift Adjust?

A year or so ago, I noted that our '98 Caravan would be "stuck" in 2nd gear when accelerating. Playing with the shift lever slightly would show that it wasn't in fact fully in "Drive". It was off by a small fraction, but enough obviously to affect the four speed auto transmission. More recently, regardless of the shift lever position, the van appears to rev high in 2nd before 3rd engages. Likewise, it appears to downshift into second too quickly when de-accelerating. Are the two instances above related? Any idea what could be wrong?

PS...
When this first started happening, "playing" with the shift lever slightly would put the gear properly in drive, and auto shifting was then normal. Van has 189 Kms at present...
