1995 Dodge Caravan after driven 10+ miles, van will jump in

1995 Dodge Caravan 6 cyl Front Wheel Drive Automatic 100k miles

I bought my 95 3.3L Dodge Caravan used from older man several months ago. Recently the van has be having mechanical problems. After vehicle is ran for 10+ miles, it will jump out of high gear while moving and down shift into low gear. Once it does this, it will get up to speed, but isn't in the correct gear for that speed. In other words, after the van warms up, it will only drive in 1st gear and that all. What could be the cause of this? A man I spoke to suggested changing the vacuum hoses and gaskets. Do you think that could be the problem or what else should I do. Money is a problem so hopefully its cheap. Thank you and hope to have a answer soon.
