When driving or idling the cooling fan does not work

I started having this issue and have been trying to fix it for 2 weeks. I had originally taken it to Ford and they replaced the fan and radiator saying there was a pinhole leak but the problem still persisted.

The only way I discovered to make it work temporarily when idling is unplugging and plugging the power to the fan control module. I had also changed the cooling sensor to see if that was the issue. Didn't fix either.

The second dealership said the PCM is showing 3 volts output when it should be 0? Not sure if this is true as I don't see anything online about PCM causing issues with the cooling fan. Also checked all the wiring with a multi-meter and there are no shorts.

Any advice or ideas are greatly appreciated.

The first step in this issue would be to retrieve the trouble codes from the power-train control module to see what is preventing the fans from operating. The codes will give us a path for pinpoint tests.

They ran all kinds of codes and had none. That's why it's such a weird issue.

A faulty ground or fan controller are the only two scenarios that I can think of right now, but to not set a code, a grounding issue is more than likely. Also inspect the fan connector and pins for heat damage or loose pins. Was the vehicle in any kind of collision? If the fans come in contact with the radiator the excess current draw can shut things down temporarily. Just a few thoughts from things that I have seen in the past 27 years with Ford.
