coolant reservoir near the top?

why does coolant come out near the top of the reservoir? and the engine not overheating and why the fan run all the time?

There could be combustion gas in the cooling system. A block test can answer this question, there are affordable ones at the parts house or harbor freight. Using the fluid if it turns from blue to green you have a bad head gasket. Just because it isn't overheating visibly doesn't mean it's not overheating.
