Car was sitting for about three months started it up and was driving, parked and left it idle twenty minutes. Noticed temperature gauge read hot. Got home and added water and saw water coming out behind back of engine. Could that metal pipe have rusted that is connected to the hose? Or maybe a freeze plug?

Hello, I am Danny.

I would suggest using a cooling system pressure tester to double check for the location of the leak(s). It pressurizes the system and any leaks would be noticed. If you do not have a tester your local auto parts store (Pep Boys, AutoZone,O'Reilly's) have a tool loaner program that rents. I have attached a link showing what is involved. Hope this helps and feel free to ask any questions if needed. Thanks for using 2CarPros.
