Coolant leak

For two weeks, instrument cluster says top up coolant level. See manual. It tops out with 3/4 gallon. Last night after fifteen minutes of driving on freeway, smoke issued from under hood, and on opening it at home, I saw water pouring from upper hose, like a faucet.

If the leak was the hose itself, it will need replaced. Here is a link that discusses how to find a leak as well as how one is repaired. If you page down a bit, it comes to the radiator hose.
