Coolant fan not working properly and randomly makes an excessive noise

Coolant fan at various times and more frequent lately will make excessive noise and seem to be stuck in high mode sometimes even when I first start it. I'm not overheating so what's the fix? Is there a way to test the engine coolant sensor or tell if the thermostat is stuck open? Or a way to tell if the fan controller module is bad. Keep in mind that I really do know about vehicles and reading meters. The car runs fine but that noise!

The noise is telling you the fan is going bad, which is common. We should do a CAN scan which is easy, you can get a CAN scanner from Amazon for about $30.00. here is a video to show you how and a link to a scanner from Amazon:

This will tell if the fan controller or coolant sensor is bad, I would replace the fan first to see if that fixes the problem. Here is a generic guide and the specific instructions in the diagrams below to show you how on your car:

It also threw a P0128 code, and the temperature gauge seems lower than usual which led me to believe it was the sensor, or the thermostat stuck open.
