Codes P2196, P2198, P0401 and P0300, cannot get it to pass emissions?

Codes p2196 and p2198 and p0401 and p0300 (all intermittently). I've cleaned the carbon out of the EGR valve. I've checked EGR pressure sensor feedback was working, hoses good, changed bank 2 sensors 1 and 2 and that cleared it for a bit now it's back, and the bank 1 sensor code is popping up and the exhaust gas recirculation is insufficient. I may or may not have a leak in the canister purge valve. IM readiness all passes except for catalyst, then all the codes will pop up again (typically after 50 miles, furthest has been 100 miles). Does not overheat, topped off coolant, changed oil and filter. Have not changed PCV valve, spark plugs or fuel injectors.

Can I ask if the engine has exhaust leaks? This will make the engine run rich, Also, I would pull out the spark plugs to see if one or more is a dark color. This will show you the fuel injectors that may be stuck. Please go over these guides to help you see what can cause the engine to run rich:

P2196 and P2198: O2 Sensor Signal Stuck Rich (Bank 1 and Bank 2)
P0401: Insufficient EGR Flow
P0300: Random/Multiple Cylinder Misfire Detected

Also, it looks like the EGR valve passages are clogged. I would remove the EGR and inspect the passages, also it can be a bad EGR valve. This video can help you test it:
