Soft clutch pedal and transmission not shifting into gear

This is most likely a slave or master cylinder issue but basically the clutch is worn out and needs to be replaced. You may get away with replacing the slave cylinder but if this is in need of replacement then the clutch is going to need to be replaced as well and it is better to replace them all at the same time.
