2005 Chrysler Town and Country 4 cyl

I have a 2005 Town & Country - The code PO406 is showing up for this vehicle. What does this code mean and what could be some of the possible causes for it?

This code is:

P0406 Exhaust Gas Recirculation Sensor A Circuit High

The EGR may need cleaned or replaced. Please discuss this with your mechanic. I believe the computer needs to be flashed. TSB 18-010-04 (PCM Flash Reprogramming). Both repairs must be performed before the engine is started
