2002 Chrysler Town and Country Car keeps sliding even in Pa

Hi,
I have 2002 Crysler Town and country,
Since last 2 months my car keeps sliding even if I put it in parking. So because of same problem I need to use hand brake.

At the starting of problem (Before 2 months) it was having cracking sound also, but now a days it doesnot sound.
Please advice me what can be a porblem.

Need to check the linkage/cable for the shifter, sounds like something has broken or worn out and the trans isnt being put all the way into park
