1998 Chrysler Town and Country rough idle

My Chrysler has a ton of miles and has been great up to now. When I stop at a sign or light it acts like it wants to die. I have noticed too that it is really rough driving on an incline specifically in the 45-65 mph range. We have checked all the plugs and changed the water pump and fuel filter. What else should we be looking at that could cause this problem?

Engine running rough or misfiring can be cause by the following:

1.Defective spark plug
2.Inadequate spark/coil,defective spark plug wire.
3.Lack of compression
4.False air leakage.
5.Faulty fuel injectors.
6.Insufficient fuel pressure.
7.Contaminated fuel.
8.EGR valve that is leaking.
9.Oxygen sensors.
10.Throttle position sensor.
11.Idle air control valve.
