2003 Chysler Grand Voyager Slips to neutral while driving

Why does the tranie shift itself into neutral while driving in D. While drining ion the highway, the tranie went into apparent neutral. The engine raced with no drive engagement. After stopping and shifting to Drive, the van started slowly with the engine racing somewhat. Soon everything was normal. This was a few days ago. It has happened a few times but rather random. What is going on?

check tranny fluid and check for codes
