1995 Chevy Van Jerking/jumping when put into gear.

After I start the van, the first time I put it in gear it jerks or jumps forward. Also, when it shifts into the next gear it does the same, only not quite so bad. After I have driven for 15-20 minutes it seems to smooth out and at times goes completely away. I had the transmission fluid and filter changed - no help. I had a rebuilt transmission put in at about 90,000.
