2000 Chevy Truck vibration at 40 mph

2000 Chevy Truck V8 Two Wheel Drive Automatic

I have a 2000 1 ton Chev, 350, auto, with a 10 foot dump box. The truck has a vibration at 40 MPH with a load and without. I have replace the ujoints and balanced the drive shaft, placed it on jack stands with and without the tires on, and check the axles for bends. It vibrates in 2nd, 3rd, and 4th. I have the diff cover off and everything seems to be okay. (bearing are smooth and no excessive play) Any ideas?

I remember a recall on that truck, it was a bad output pushing on the transmission check your driveline at the transmission up and down make sure there's not excessive play at the transmission housing, also check the rear differential yoke make sure there's not excessive play there either.
