At 45mph under very little load just cruising I get a what feels like a bog or bump never under 40 or after 65. Have done intake manifold gasket, O2 sensor, new distributor plugs and wires. I'm at a loss any ideas?

If you are positive it is engine related, then have ignition module and coil checked.

This will be difficult to isolate since it happens only under specific conditions.

I drove the truck for a long period of time, its only a second car and found it only does the bog or bang in overdrive runs fine in 3rd must be a tranny issue. Shifts fine no other issues do u think it could be a lock up solenoid problem or at 140k the tranny is going?

Yes could be the TCC solenoid. Some are replaceable outside trans. Check with local trans shop. Transmissions are very complicated and you need a pro to diagnose it.
