1994 Chevy Suburban Servo cover

I have a 1994 Chevy Suburban 4 wheel drive with automatic transmission. What would cause the servo cover on side of transmission to push out and leak all fluid out? It did this the first time while passing another car. The second time just going down the road.

There are possibilities that the ring is not being fully engaged in the groove, the ring is deformed or wrong, the groove in the case is worn and not holding the ring. I have also seen the case broken around were the depression(groove) is.
