2006 Chevy Silverado Fan

The AC fan will not come on. The compressor clicks on. The light on the dash lights up indicating that the compressor is on. This happenned once before, but lasted only one or two times that I re-started the ignition. The fan has 5 speeds, but the 5th speed has not worked in quite a while. If the fan motor has frozen, where is it located?

The motor is located under the dash on the passenger side. Test for power and ground arriving at the motor..
